HMIS Client Release of Information
Posted Data Privacy Notice
Agency staff should complete the HMIS Client Release of Information form before the client data is entered in HMIS. This form should be completed with clients yearly.
The Posted Data Privacy Notice must be displayed in your agency's client intake area.
Please email feedback, suggestions, or new form requests to hmis@cohhio.org.
Upon request, any client may be provided with a list of agencies who share data in HMIS. This list is updated annually. Please direct questions about this list to hmis@cohhio.org.

Notify COHHIO that a new homeless-dedicated project is beginning to serve clients in the Ohio Balance of State CoC or Mahoning County CoC
