1.1. 2017 Annual Renewal Instructions

1. All persons wishing to become or remain HMIS End Users, including those with Read-Only access, must perform the following steps:

2. One person from each HMIS participating agency should complete the HMIS Agency Agreement and Agency Administrator Agreement.

3. All completed HMIS End User Agreements, the HMIS Agency Agreement, and the Agency Administrator Agreement should be emailed to hmis@cohhio.org .

For each agency's associated HMIS providers and End Users to remain active, all of the above steps must be completed by 09/01/2017. If they are not, the agency's End User accounts may be inactivated until the steps are completed.
